How to Become a Patient of Flourish Health


Many patients seek out Dr. Luetmer because they have been bounced around from provider to provider only to receive "band-aid solutions" and have finally resolved to identify and address the root cause of their health challenges. Other patients come to Flourish because they are generally healthy and want to maintain and build a healthy lifestyle and understand how functional medicine complements the goal to remain as healthy as possible and avoid chronic diseases. 
The research required and mutual commitment to a plan lends itself to a model the fosters an ongoing patient/physician relationship. We built Flourish specifically for that reason and we offer membership plans to address the ongoing needs of individuals and couples. Flourish also serves many families who may have one or more members facing health challenges that require “digging in” as well as other families who are generally quite healthy. Given the variables of family size and needs, the fee for an initial family consultation is determined after speaking with Dr. Luetmer to set up an appointment of the appropriate length. Dr. Luetmer can serve the health needs of an entire family, performing well-child physicals, sports physical, etc. as well as the more complex health needs that might be going on with some family members.  Dr. Luetmer can also serve as the family’s primary care physician.

The Flourish Approach

THE PROCESS OF BECOMING A PATIENT OF THE CLINIC
Since starting Flourish in July 2019, we have continually fine-tuned the new patient onboarding process with the goal of achieving the best possible results while containing costs. There are three phases to becoming a patient of Flourish Health: the Initial Assessment, the Intensive Onboarding Period, and Annual Membership. These three phases are designed to flow into each other in unbroken continuity in order to maintain momentum and forward progress on your health plan. That said, patients may "opt-out" at the end of any phase if you are not achieving your goals or simply feel Flourish is not a good fit for you.


Phase 1:
The Initial Assessment

This phase lasts 4 weeks beginning with the completion of your Case Review. The fee for Stage 1 is $700 for an individual or $1,100 for a couple. Appointment times for couples are lengthened from what is listed below. No fees are incurred until after you submit your case review and we mutually agree that Flourish is a good fit for you.

  • The Readiness Assessment and Case Review: ​You will start by taking a short readiness assessment. If you have not already completed this, the link is above. The readiness assessment is designed to measure your readiness to make lifestyle changes that may be necessary to achieve your health goals. This will be followed by completing a comprehensive medical history and self-assessment, which we refer to as the case review. The case review will likely be much more thorough and unlike those you may have completed in the past and is designed to dig below the surface so you can work with Dr. Luetmer to identify the root cause of your health issues. 
  • Access to Member Portal: Once you complete the case review, we will create a portal account so you can access Dr. Luetmer's schedule to book your first appointment and communicate with her and our health coaches through the portal.  Dr. Luetmer will review your case prior to your initial phone appointment to confirm there is a fit with her qualifications and the practice. If she does not think that she can be of help, we will let you know and you will have no financial obligation for this review and any payment made to that point will be refunded.  Below is the general timeline for Phase 1, which can vary slightly depending on which labs you might have drawn and the length it takes to get results from some of the specialty labs.
  • Week 1:
    • Obtain additional health history (recent labs and/or MyChart or similar portal access)
    • 30-minute phone or zoom call with Stacey
      • Hone in on symptoms and health history
      • Determine whether additional assessments, questionnaires, and/or labs will be beneficial.
  • Week 2:
    • ​Lab draw
    • Watch assigned educational content from member site
    • Complete additional questionnaires
  • Week 3:
    • ​60-minute coaching call or zoom with our health coach.
  • Week 4:
    • ​60-minute in-person appointment with Stacey
      • Begin Health Plan Development
Phase 2: Intensive Onboarding Period: The next phase of your health journey is focused on building out a comprehensive health plan that you can successfully manage going forward. An effective health plan is not a "cookie cutter template" that can easily modified for each patient over the course of 2 or 3 appointments. Everyone is different and it takes time to implement changes, measure responses, and make necessary modifications to some of the initial recommended steps following your appointment in Week 4 of the Initial Assessment.  This 8-week process will be supported by (4) 30-minute coaching sessions and (2) 30-minute appointments with Stacey. In addition to scheduled appointments, you will also have access to the Flourish Member Portal, which provides access to submit written questions to Stacey and our health coach(s) for support between appointments. This phase is billed at $299/month. In some cases, a third month of intensive onboarding may be necessary. 

Phase 3: Annual Membership: The journey to sustainable good health is a process that requires ongoing effort and commitment. The Flourish Membership is designed to provide the ongoing support, educational resources, and networking with other members to increase your odds of long-term success. 
  • Toward the end of Phase 2, (week 10 or 11) you will receive an email with your recommended level of membership based on Dr. Luetmer's assessment of your treatment plan progress and support ticket activity. 
  • Membership will automatically start on week 13 at the recommended level unless you specify otherwise.
  • You are not obligated to have an annual membership with Flourish if you proceed with an Initial Assessment; however, over the past 3 years, the vast majority of Dr. Luetmer's patients (roughly 95%) have chosen to do so, which is why we have designed the process to roll right into membership. At the end of Phase 2, your options are to stop care all together, choose base membership (see below), or proceed with Dr. Luetmer's recommendation. 
  • Based on our experience, most members are able to downgrade their plan in year 2 as they make progress on their health goals and require less support going forward. ​

Base Membership

Base membership benefits are included as part of all Full Membership Plans (Bronze, Silver, Gold, & Platinum). Base membership is also available as a stand-alone benefit ($45/month). At Flourish, we spend a great deal of time and energy negotiating the best price on health services and products on behalf of our members and those savings are passed on to you. Members enjoy perks such as discounted pricing on labs and wholesale prices on supplements.  Members also receive access to Dr. Luetmer’s online education courses, group meetings, and other benefits.

Full Membership

In addition to the benefits of the base membership, patients with full membership also have access to Dr. Luetmer and our health coaches throughout the year via in-person visits as well as telephone and Zoom (virtual) visits. Members also have access to our member portal through which they can securely communicate their health questions and retain a log of all their communication with Dr. Luetmer for future reference. Bronze Members can use their credits for all non-urgent visit. Express care visits will be billed at the standard rates.
Silver Members receive all Bronze benefits as well as have the ability to use credits for urgent visits.
Gold Members receive all Silver benefits as well as access to priority appointment slots.

This membership level is designed for individuals already familiar with functional medicine working on one or two main issues or relatively healthy patients seeking a functional approach who want to tweak and/or “stay on track” with their health journey. 
This membership level is designed for individuals who have one or more moderately complex chronic condition(s) that require a relatively consistent amount of ongoing support. Additional time is built in for more clinic visits. Couples with only one spouse experiencing complex issue(s) and relatively healthy families will also fit into this plan. Most Flourish Patients are Silver Members.
​This membership level is designed for individuals who have complex health issues and/or desire a higher level of ongoing support.  Frequent touchpoints are built in to accommodate a close partnership with Dr. Luetmer to avoid cycling through major health swings. Complex couples and larger families with one or more family members facing complex issues will also fit into this plan. 
This membership level is designed for individuals who have multiple and very complex health issues and/or desire unparalleled access to a functional medicine MD.   Due to the nature of this membership, the Platinum level is limited to 10 patients. 

Additional Membership Details

What are Member Credits?  

Membership credits primarily translate to consulting time you can “spend” as part of your membership and provide you with the flexibility to connect with Dr. Luetmer and our health coaches, using the avenue that works best for you.  For example, some people prefer using most of their time for in-person appointments; whereas others like to spend more of their credits toward online support to ask ongoing questions of Stacey or our health coaches.  1 credit equals 1 hour of consulting time with Dr. Luetmer, whereas 1 credit equals 2 hours of consulting time with a health coach.  

How do I Maximize my ​Credits?

There are a number of member benefits that do not reduce your credit balance; from member group events to live Q&A educational seminars. In addition, participating in group clinic visits to discuss health issues and strategies common to the group will only deduct a fraction of a credit. For example, group visits that generally last between 60-90 minutes only “cost” .25 (¼) of a credit. A unique aspect of Dr. Luetmer's practice is the 200+ hours of video education she has produced that is available to members only. This education is designed to elevate a member's understanding of their health issues. Stacey "prescribes" specific videos to patients so members can maximize their appointment time since Stacey is not wasting time "covering the basics" that are addressed in the video education.​
